FOREIGN LABOR CERTIFICATION REQUEST FOR H-2B JOB ORDER

This job order request is being placed in connection with a future application for H-2B workers.  
The ETA Form 9142B must be filed with the Chicago National Processing Center at the same time you submit this request to Missouri Foreign Labor Certification.

EMPLOYER ASSURANCES
A single workweek will be used to compute wages due.
The employer will provide workers all tools, supplies, and equipment required to perform the job at no charge to the worker.
Period of Employment is 120 days or more. The employer guarantees to offer work for hours equal to at least 3/4 of the workdays in each 12-week period of the total employment period. OR Period of Employment is fewer than 120 days. The employer guarantees to offer work for hours equal to at least 3/4 of the workdays in each 6-week period of the total employment period.
H-2B workers will be reimbursed in the first workweek for all visa, visa processing, border crossing, and other related fees, including those mandated by the government (excluding passport fees).
Transportation (including meals and, to the extent necessary, lodging) from the place from which the worker has come to work for the employer, whether in the U.S. or abroad, to the place of employment will be provided, or its cost to workers will be reimbursed, in full, and subsistence expenses will be reimbursed at $12.09 per day minimum without receipt to a maximum of $51.00 per day with receipts when the worker completes half the employment period. Return transportation to the place from which the worker came to work for the employer will be provided if the worker completes the employment period or is dismissed early by the employer.
Board, Lodging, or Fringe Benefits (optional): The job order discloses the provision and cost of any board, lodging, or other facilities, including fringe benefits or assistance to be provided to the workers in securing such benefits.
Daily Transportation (optional): The employer will provide daily transportation to and from the worksite(s) from a central location to be determined by the employer.
